COVID-19 PROTOCOL

In-Person, Hybrid, and Virtual Class Offered As of July 28, 2021

Urban Artistry Dance Academy (UADA) is excited to announce that we will be offering both in-person classes on July 12, 2021 in addition to maintaining virtual classes. We understand the frustrations, uncertainties, and concerns that COVID-19 has brought into our lives. The main goal of these options is to provide our communities with the flexibility and the ability to make the best personal choice they can to begin or continue to build together.

Urban Artistry, Inc. has taken many precautions beyond MD State Health Department regulations for our in-person classes. By offering in-person and virtual classes, we can be as adaptable as possible, no matter what the year brings- and maintain relationships with students that we have developed outside of the immediate DMV area. 

 

IN-PERSON CLASSES (also offered as virtual classes)

All UADA classes will be offered in-person, beginning on July 12,2021 with the exception of the classes listed below as “Virtual Only.” The number of students allowed in the studio will abide by MD Health requirements, and we encourage early registration through MindBody to secure your in-person spot. 

In-person Urban Artistry staff and instructors have been vaccinated and are required to wear masks. The staff will clean highly touched surfaces in between classes and sanitize the studio on a daily basis.

PRIOR TO ENTERING STUDIO:

 

RESPONSE PROTOCOL: (See “What if someone at UADA tests positive for COVID?”)

Should an individual report a positive COVID case, the studio will immediately be sanitized, and encourage all exposed close contact instructors and students to get tested, follow CDC guidelines, continue with virtual classes until cleared for a healthy and safe return.

 

CANCELLATION POLICY: If you present a fever, fail to sign the “Community Safety/Accountability Pledge and Liability Waiver,” or  refuse to follow UADA staff instructions to maintain community safety, you will not be allowed to participate in UADA in-person classes that day. We encourage you to take the class virtually. If your registration has been cancelled, but you have already paid, that payment can be used for another class within a 1 year period from the purchase date.

Cancellation Policy: If you have paid for, but are unable to attend class, payment will remain in the MindBody system and can be used for another class within a 1 year period from the purchase date. 

What If Someone At UADA Tests Positive For COVID-19?

If there are any confirmed cases of COVID-19 reported to Urban Artistry, Inc. (UA) by a student, family, or staff member UADA will follow the State of Maryland and Maryland Department of Health COVID-19 requirements.

INDIVIDUAL REPORTING COVID EXPOSURE

As noted in our Community Agreement/ Liability Waiver (signed prior to entering the studio), we ask that if you confirm that you have contracted COVID, or may have exposed anyone at the Urban Artistry Dance Academy (UADA), that you alert UADA staff by email: studio@urbanartistry.org immediately. Your confidentiality will be maintained. 

Should an individual report a positive COVID case, the studio will immediately be sanitized, and UADA encourages all exposed close contact instructors and students to get tested, follow CDC guidelines, and continue with virtual classes  until cleared for a safe and healthy return.

COMMUNITY NOTIFICATION

UADA will inform community members of the positive individual who will be asked to self-quarantine and to notify MD Health. UADA will notify all students and staff who have visited UADA on days that the affected individual was at the studio and will follow all steps outlined by MD Health and the Centers for Disease Control and Prevention (CDC) for cleaning, disinfecting, and sanitizing of the facility.

When Is It Safe For A Staff Member Or Student To Return To UADA If They Have Been Ill?

COME PREPARED

Students should come to the academy in comfortable clothing and supportive athletic footwear that allows free movement.  Students should come prepared for challenging mental and physical activity, bringing ample supplies to ensure their wellbeing, such as:

Students should notify the instructors of any health concerns that may affect their participation in the class.  The instructors will work one-on-one with the students to modify movement to better match the students’ needs. It is up to the student to communicate his/her limitations and ultimately decide on his/her level of participation.

Students are encouraged to explore different styles of clothing and footwear within the different genres of dance to best compliment that movement.

CANCELLATION POLICY

For up-to-date schedule information, please check our website, visit us on social media, or check your email. Any schedule adjustments will be posted at least two hours before scheduled class times. When possible, our staff will do their best to contact individuals about cancellations and provide an opportunity to make up the missed class.

LATENESS POLICY

Late students are welcome to come to class whenever they arrive.  Please note that full payment for class is expected.

PAYMENT POLICY

Tuition is collected prior to a student taking a class. Urban Artistry, Inc. accepts cash, debit/credit cards, PayPal, and personal checks as payment. There is a $25.00 returned check charge for any checks returned by the bank.

PHYSICAL ACTIVITY LIABILITY FORM

Section 1: Contact Information
All students of Urban Artistry, Inc must provide their full legal name, email or phone number, and home and billing address(es).

If a student of Urban Artistry, Inc is a minor, the following additional information is required: the student’s/participant’s birthdate, the name and contact information for the student’s parent(s) or legal guardian(s).

Section 2 : Medical Emergency
All students and/or participants in events of Urban Artistry, Inc must provide an emergency contact name, phone number, and email.

All students and/or participants in events of Urban Artistry, Inc must communicate any ailments/ restrictions/ medical Conditions they would like us to be aware of.

Students and/or participants in events of Urban Artistry, Inc give permission to Urban Artistry, Inc, its owners and operators, to seek medical treatment for the participant in the event they are unable to reach a parent or guardian. Students, participants, and parents or guardians of students/participants may request that Urban Artistry, Inc contact a certain doctor/physician for the student/participant and that the student/participant be transported to a certain hospital. Students, participants, or their parents/guardians are responsible for providing that information to Urban Artistry, Inc.

Section 3: Customer Care
Urban Artistry strives to create classes, communities, and experiences of exceptional value. We will make every effort to notify you in a timely manner of class cancellations (through the distribution of calendars, class announcements, the website, email and social media updates).

If class is cancelled due to teacher absence, all students who have attended a class within the past month will receive notification upon notice. If a class is cancelled for reasons due to studio closure, the class schedule class will resume the following week.
If you become injured, please make this known to the administration so that we can credit your classes for up to a year from the date of notice.

In the event that you have any suggestions or concerns about class, please make it known to the instructor or administrator directly. We will make every effort to implement your suggestions. Section 4: Release of Liability
I acknowledge that dance is an athletic activity and such activity is subject to mishap and even injury to participants, including the potential for permanent paralysis and death. I understand and acknowledge that dance has inherent dangers that no amount of care, caution, instruction or expertise can eliminate this risk. I expressly and voluntarily assume all risk of personal injury sustained while participating. The students or participants in events of Urban Artistry, Inc. release and hold harmless Urban Artistry, Inc., its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant, while in or upon the premises or any premises under the control and supervision of Urban Artistry, Inc., its owners and operators.

REFUND POLICY

All sales are final. If you become injured, please make this known to the administration so that we can credit your classes for up to a year from the date of notice.
